This benefit eligible position is Monday - Friday, 8:
00am - 4:
00pm.
Must have valid drivers license and good driving history.
Must have reliable vehicle.
Must be able and willing to drive all program vehicles.
Successful candidate must pass CORI/SAFIS and Driver History check.
Direct Care Staff provide support services directly to adult individuals in the day program, at their places of employment, and/or in their residences.
Assists in the implementation of the Individual Service Plan (ISP) goals including learning routines of daily living, facilitating inclusion in the workforce, and driving individuals to/from employment sites, programs, and residences.
Essential Duties:
Assist in the implementation of individual service plans and behavior intervention plans.
Maintains a culturally diverse, purposeful, respectful, and stimulating program environment that is organized and positive in nature and geared towards consumer needs and interests.
Follows schedule as assigned including reassignments to different program locations as needed.
Prepares for, attends, and fully participates in staff meetings, and individuals meetings if requested to do so by administration.
Drive program van and/or personal vehicle as needed for community activities and transport.
Accurately completes documentation of progress notes, medication logs, and incident reports in accordance with DPH/DDS requirements.
Complete all expected paperwork in a high quality manner within mandated/required timelines.
Shows respect and compassion for all members of the program including consumers, families, staff, and other community members.
Provide individualized support to consumers, which may include activities of daily living (ADL's) such as toileting, feeding, transferring, self-care, and assistance with mobility including lifting.
Effectively utilizes any recommended adaptive equipment for individual consumers.
Communicate all significant occurrences to the appropriate supervisor in a respectful and timely manner.
Maintains professional boundaries with families and consumers.
Administer medications in accordance with MAP requirements.
Expected to read and adhere to all Employee Handbook policies.

The check will bounce and the victim is left responsible.
